At Cornerstone Hospice and Palliative Care, patient care is provided within the familiar setting of wherever the patient lives and calls home:  his/her own home, assisted living facility, nursing home, independent care facility, Hospice house, or inpatient unit.  However, the patient does not have to be home-bound.
